Goal: Teach students end-to-end investigation and facilitation skills for leading a small RCA team or completing an individual investigation.
What will you learn in this training?
- Facilitate a root cause analysis based on evidence and causal logic
- Gather and manage evidence data
- Create a detailed problem statement
- Analyze cause & effect relationships by building a cause & effect chart
- Identify effective solutions (corrective/preventative actions)
- Recognize and correct systemic causes
- Complete multi-part RCA exercises individually and as a team
- Remove focus from generalization, opinion, and blame
- Shift focus to evidence, logic, causes, objectivity, and collaboration
